Your first visit to Tarzana Smile Design establishes a vital foundation for our
relationship with you. During the first visit, we make sure to obtain important
background information, like your medical history, and give you time to get to know
your doctor. To understand what to expect for your first visit to our encino practice,

first visit by printing out and completing the patient forms in advance of your
appointment.

We are dedicated to providing you with the most caring and highest quality dental
care! Our entire dental team is committed to making each visit at our office
pleasant and comfortable, by taking the time to listen and treating you as an individual.
Our Encino rooms are all equipped with televisions, personalized headphones and
comfort pillows, which along with Nitrous (laughing) gas and twilight sedation,
will allow you to reach the level of relaxation that is right for you.
By continuously staying up-to-date on the latest advances in dentistry, and by using
the highest quality dental materials and equipment, you can rest assured that you
will always receive the finest care. Whether your dental needs are simple
or you are looking for an entire cosmetic make-over, you will always leave our office
with a comfortable, healthy, beautiful smile.

Dental Appointments
In order to provide excellent, timely care to all our patients, we ask that you
be on time for your appointments. If for any reason you cannot keep a scheduled
appointment, or will be delayed, please call us as soon as possible.
Insurance and Billing
We accept most traditional insurance plans, contact our office to verify acceptance
of your plan.
Tarzana Smile Design does not participate in Health Management Organizations; however,
we will be happy to file your insurance claims for you. We accept checks, cash or
credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator
for details. We are happy to file insurance for your reimbursement as long as you
are free to choose your own dentist.
Financial Options

1.Financing
Tarzana Smile Design offers financing to all patients upon application and approval.
Multiple financing options are available, including 12 month interest-free financing
and low monthly payments for up to 60 months.
If you would like to apply for financing online, please use the link to CareCredit.
Otherwise we would be happy to take care of this for you in our office.
2. Payment at Time of Service
Though most of our patients prefer to take advantage of our prepayment discount
to reserve their appointment, you may choose to make payment the day treatment is
started. No discount will be applied, nor financing arranged for this option.

the needed information to provide the best possible care for you. It also will help
relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms
